my mom used to say that mayb the baby slept in the wrong position.. probable your four year old slept on his left for long and this may hv put some pressure on his neck.. so, probably just a muscle strain.. try this.. whenever he's asleep, just move him side to side.. like if he's sleeping on the left, move him to the right and vice versa.. also, put some baby oil on his neck and shoulders and massage it gently.. if it still persists, then mayb u should get a second opinion..

When I was a kid, I used to sleep in weird positions and wake up crying because of the pain I was in.
Maybe it's only that?
Even now, sometimes I wake up with a very stiff neck and can't turn my head the direction the pain comes from.

But since this wasn't told to you when you took him to the ER, it probably rules my idea out. :-\

He is either sleeping with his head turned, or he held his neck in a position for a long time the day before he complained of pain.
