In life, do not take things literally. People say lots of things which they dont mean or say something but mean something else.
So, just try and understand what they meant. If you are not sure then give them benefit of doubt and say "I am doing great, how are you"?

Depends on who the person is. If it's someone who I think actually cares, I tell them. If it's a casual aquantance, I usually wont.

well concidering i got no balls..they dont ask me that if they do i grab my boobs n shake them n say "how do they look to u" i am positive they would say " hella great"
