"Intends to" refers to the one doing the intending ...

"John intends to do his homework."
"The lion intends to have you for lunch".

"is intended to" refers to something other than the one doing the intending. It refers to the object of the intending, not the doer of the intending.

"Food is intended (by the mom) to be eaten, not played with".
"Homework is intended (by the teacher) to help a student learn".

"Intended to" means that in the past I was going to do something and I did or did not.
"Intending to" means I have the intention of doing something in the future. I may or may not.

They are two completely different tenses. For example "I intended to take a huge dump on my girlfriend's lawn, but had a milkshake instead." and "She intends to take a dump on my lawn, and because she's really friggin' crazy, she might do it".

It's about who intends.

Active voice: "He intends to work."
It is his intention.

Passive voice: "He is intended to work."
Someone else expects it of him.

is intended to means that the person meant to do what they did, intends to means that they are going to do it.
