If you're cooking the beef, and have the chicken laying on a cutting board of some sort, then you take the beef out and put it on the cutting board so that you can cook the chicken - Voila! You've just contaminated cooked beef!
Also, if you've touched the raw chicken, and then you touch the beef - it can happen.
Or, if you use the same utensils to prepare the chicken, and then to do something with the cooked beef.

A hypothectial restraunt kitchen serves a mixed menu of poultry and beef. In this kitchen there are several cutting boards and utensils used to prepare the food.
Worker 1 uses a specific cutting board and knife to slice up some chicken prior to cooking. Instead of cleaning the cutting board properly worker 1 simply rinses the board off with cold running water.
Worker 2 comes along and has a serving of cooked beef that needs to be prepared and finds the cutting board that appears to have been cleaned properly. Worker 2 then uses that board and the contaminates on the cutting board left over from when worker 1 used it is now attached to the cooked beef.
This is but one example but I believe shows how when proper preperation techniques and precautions are not followed it can have diasterous results.

By using the same surface to cut the raw chicken and then to cut the cooked beef. Or by using the same fork to pick up first the chicken and then the beef without washing it in between.

Raw chicken could drip blood on the cooked beef mistakenly stored below it in a refrigerator.
Someone could use the same tongs that put raw chicken on a cookie sheet to serve cooked beef in the cafeteria.
There's a jillion possible situations!
