I am french and just to let you know that the right answers among all are:
Comment vous appelez-vous? formal as previously said
Comment t'appelles-tu? and familiar
(with a little "-" that has its importance if you want to be perfectly correct)
I would not say "Quel est votre nom?" unless I am working in an office and need to write the name of the person down. It is not the way french people generally ask the name.

You can also try www.translation.com. This site allows you to translate English to almost any other language, and vice versa. I will admit that it isn't much help with slang words, so most of the translations will be formal.
