How do you check sensations in any patient? Your question reflects that you haven't been paying attention to physical diagnosis. A diagnostician must be prepared to understand the condition of the patient without a word spoken....if he is good....Dr. Sodeman could stand at the foot of a bed and diagnose a patient without even touching him....see Sodeman's book 'Pathologic Physiology'. You need to use your brain, and all of your senses, smell, touch, hearing, seeing...and while we don't use taste today, the first gynecologist used to taste the urine of their patients. You check reflexes, use pin-prick, use gentle touch.....if you need more information to do a simple task such as this, you will be a shabby physician.

Aphasia just means that person cannot speak or cannot say the words they want to. It does not mean that they cannot respond by shaking their head or in other ways. If someone hurts they can still groan or say OH real loud or some other noise. They can smile if they like something.
