You should have your current driver's licence, a copy of your birth certificate, social security card, proof of residency, and some states will require you to show proof of insurance.

You need your current driver's license and proof of your new address. Most states will also give you a written driver's test when you apply although you have a license from another state. It is also a good idea to take along your birth certificate since they will probably want another form of ID.

Every state has Department of Motor Vehicles offices in numerous places. When you move to a new state, you need (usually within 30 days) to take your old license to a new state DMV office and apply for a new license. They will accept your old license as proof of your identity, but might require some evidence (such as a utilities bill) of your address in the new state. You fill out some paperwork, give them some money, and you walk out with your new license (and probably also with your old one with a hole punched in it).

You need to have your old driver's license, provided it is still valid, you will also need proof of your new address, and your birth certificate, and SS card. Good luck. You will also have to take an eye exam.
