I am not sure how you are asking this, but I think you are asking how. It is by touch and talking. You can feel a person tense up and relax. You can also find out what people like from their families. Nurses are special people who have very good intuition, which helps in cases like you mention.

I'm not sure, but I would guess that the nurse and patient would write things down. If the patient cannot write, they may need to do a "blink once for yes, twice for no" type of thing.
