chronic is constant, you're not going to die from it. Terminal means you're not going to make it through. Death is the ultimate end.

Chronic disease is opposite of acute disease. It is a disease that u have for a long time, it may shows symptoms, or it can be silence. eg: diabetic, gout, etc. It can become acute if u get the attack once in a while....eg.. u get swollen toes due to gout... =acute on chronic gout.
(I know u did not ask about acute, but mentioning it, make it easily understandable).
Terminally ill, nothing can be done. If the doctor did perform anything, it's just to ease discomfort while waiting for the 'time' to comes. Death is sure...(unless miracle takes over..)

Chronic is a disease that you can not get rid of, however it will not cause death. Chronic diseases can get better and worse, such as allergies. They may be fine in the winter, but spring makes them worse. A terminal disease is something that you will die from, usually it will be in a fairly short amount of time.

a chronic disease you will have for the rest of your life, but can be treated to lessen the problem, and keep it from getting worse. a terminal disease you will dye from, untreatable, just make you more comfortable. a chronic disease can be terminal in the long run

Chronic disease is treatable with whatever the doctor subscribes.
Terminal pretty much speaks for itself.
