A swollen gland will go away with medication or time depending on what is causing the swelling due to illness. A lump will stay.

I think you'll find with a swollen gland you will have other symptoms, ie: general feeling of unwellness , glands in other areas also swollen (underarms, groin). Throat will feel scratchy,hard to swallow etc....
Most lumps of concern generally exhibit no pain or symptom and are detected accidently.

You can have swollen lymph nodes, swollen salivary glands, and nodules in the thyroid centrally. Nodes occur at the side of the neck and in chains. All neck masses would fall into one of these categories, and not just a lump.

If a gland is merely swollen it would still be palpable. Lumps are hard and don't "move" .

a swollen gland will appear very quickly, and go away.
a lump will appear gradually, and gradually get bigger, and won't go away. hope this helps!
