There's several ways. You need to consult some books on the subject. Most "introductory to witchcraft" books will explain this.

Your altar can be made of virtually anything. Set it up so that you (facing the altar) are either facing north or east, depending on your tradition. Cleanse and consecrate it. Place your consecrated tools on it. They can be in ANY order on the altar, there is no "orthodox" way to place them. Make sure you have an athame or a wand, a chalice, a pentacle, statues or candles representing the God and Goddess, and an incense burner, at the very least.

Find a flat surface. If you're going to leave it in one place, it can be heavy (large stones, etc). Otherwise, pick something portable (even if just a folding tray table).

Make a note to use it for your altar and not to use it for anything else. Or use it for everything and just use it as an altar when you need it. Really doesn't make much difference.

You can use anything as an altar. Portable table, coffee table, end table, a tree stump, a rock or you can make one out of almost anything. You can make a permament one or portable one.
