Go to the library and sit way in the far corner where there are absolutely no distractions. Don't sit near a window or anything. Then you are forced to study. Start at 30-minute study blocks, then work your way up to longer periods of time.

You could try 20 minute intervals with a 5 minute break and then go back to studying. The key is to find that balance. Also, working on what time of day you like to study more is good too. Do you study better in the morning before school or late at night, in the afternoon?
What I've managed to do is find out how much I need to study to get the grade that I want in a given class. Say, for History, I don't care if I only pass, so I just study enough to pass. For Math, I study more because the class matters more. That's just me. I don't really recommend that way. Studying with short breaks seems to be the best way. You can also start studying for test earlier than the day before so that you don't have to study for a long period of time in one day.

Just practice studying. Study for half an hour and then increase it by 5 minutes a day. Or study for half an hour, take a 5-10 minute break.

After you finish sitting down for more than 30 minutes (or however long you want) just reward yourself or something. Go play video games. Or do something you like to do, but never have any time for.
