I think people are confusing heartworm TREATMENT with heartworm PREVENTATIVE. The latter IS a little pill that can be given to prevent heartworms, but if your dog already HAS them and has to be treated...I have to say that is it not the most pleasant thing. There will probably be a patch of hair on his or her back that will be shaved, and then the Vet gives injections in order to kill the heartworms. Heartworms are exactly as they sound, long worms that infest the heart, and can be life threatening...so even if you think your dog wouldn't like the heartworm treatment (they usually give some type of pain patch to be applied to the skin while they are getting the treatments), the heartworms are WAY worse. I have seen many dogs after their heartworm treatments, and they are back to normal in no time. Good Luck!

This pic will give you some idea

With the treatment the idea is to kill these worms in a way that when they die and brake up into pieces they will not block arteries causing strokes and the like. So the very best treatment for heartworms is "Prevention" so that treatment never becomes necessary.
Some people find heartworm medicines to expensive,so they take a very unessary chance. Because for most dogs (not some Collie type breeds) a 100 year supply of heartworm medicine can be made for aprox $20.
Here is a link on how to make it

No heartworm treatment is not painful. It can have side effects though. Your vet will check out your dogs health before administering the treatment and your dog should be hospitalized while receiving the treatment. Call your vet.
