he's got a concussion, and he needs actual medical help. Either get him to an ER or to an urgent care facility.
With a concussion, he shouldn't be taking pain meds because they can mask other symptoms - this causes lots of problems!
There can be lots wrong and you are risking his life!
Get him in the car, and say you're going somewhere other than the ER - but get him there.

he shouldn't need to go by ambulance, since it has been two days, however, the problem lies in the constant headaches, those shoudl be subsiding by now. He does need to go to the doctors however, is there a 24 hour walk in care facility that you could take him to? If no, do take him to the ER by car. He needs to be seen to rule out anything severe. Convince your friend that your are concerned that there may be something wrong, and that YOU would feel better taking him. Press the point that it would be a favor to you, this will help him get past the machismo of not visiting the doctor and can justify it by seeing it as a favor to you. Also, do not take too much tylenol, the benefit of tylenol is not linear and more relief will not come from eating more than the recommend dose. Liver failure may though. . . .

Yes, call an ambulance. He may have concussion, brain damage, major nerve damage, damage to his eye...the longer it's put off, the less likely it's possible to fix the damage completely.

Sonnds like he might have a concusion. I think you should take him to the hospital, and then find a better friend.
