Why can't you download the form if you have access to a computer? Or you could have your form faxed to the nurses station of where you are.

Okay? I have not a clue about that, but I would think that your relative or friend or someone at the hospital could get that for you. If it is for insurance, you could call your agency and have them fax the paper there.

Why don't you give it to the nurse and she can give it to the doctor, or have it ready when he comes in for rounds and have him fill it out?
