The five points of Calvinism were correlated to answer the five points of Arminianism at the Synod of Dort. Calvin was not alive or present at the council, and never made reference to five points in his literature. The five points did not originate with Calvin either, they are found in form in the writings of Augustine, and I believe, founded in the Scriptures.

People who call themselves "Calvinists" are not followers of John Calvin. Calvinism, is simply a term to describe a set of theological beliefs. Synonymous terms include, the doctrines of grace, and reformed.

In brief, and as has already been set down, the five points are these:
Total Depravity (or total inability). This means that man is tainted by sin, and unable to change his condition otherwise. It does not mean that he is incapable of doing any good at all, but the moral good that he does, as it is not done with a mind to bring glory to God is iniquituous, and therefore sinful.

Unconditional Election. This means that God, before even creating the universe had chosen a great number to be His adopted children. It is not based on His knowledge of their acceptance of Him in time, but on for His purposes and glory alone.

Limited Atonement. This means that Jesus blood was shed soley for those whom the Father had set aside to save. It does not mean the atonement was limited in it's power, only it's extent. The blood of Christ is sufficient for the entire world, but only efficient for His elected ones.

Irresistable Grace. This means that those whom the Father has deemed to save will come to Him. His spirit will cause their desires to be changed, from running from God to running to Him. They go from being spiritually dead like Lazarus, to having their stony heart replaced with one that beats for Christ.

Perseverence of the Saints. This means that those who have been truly saved will be kept in the Father's hand until the end of time.
