Don't be so hard on yourself. If you have been with someone for a long time its not unusual to call your new boyfriend the name you have been saying for awhile. I did the same thing when I starting dating someone new after dating my ex for 4 years. I had to apologize several times, but explained to my new boyfriend it wasn't intentional and I no longer had feelings for my ex. After dating my new boyfriend for awhile (about 3-5 months) this stopped happening and I made my point.

Sounds to me like you're still in denial if you say you're not still thinking about him. You should have more control over your thoughts, if you call him by someone elses name during sex then it does mean somewhere in your mind you were thinking of this guy, which again, you're in denial of.
If I were that guy I probably would've high-tailed it after that, heh.
Tell him that you don't deserve him, and that you need some time apart because you obviously are just in a rebound relationship, and you still have feelings for your last b/f.

how long were you with your (now) ex? it's really quite normal to get names mixed up, especially when there is a long-term relationship. It may take some time to get the name right, so my advice is, when you are having sex, say something generic, like "oh, baby" instead of using his name. Less ego-blasting for him that way.

if you call your new bf by your ex name it mean you are still thinking about your ex and if you and him is having sex and you call him your ex name you won't have to worry about that new bf because he will know you still have feelings for your ex.

Hey thats human nature!Just explain to him that you call everyone someone elses name at some time,who doesn't?Right?I do all the time,i mean my dad even calls me dear on accident cause he's so used to calling my mother that,of course he corrects himself,but we all do that.Tell him he's not the only one,that you mistakingly do that to.
