It really depends on the service:
At the ambulance company where I work at, we have "shift truck medics" who work 24 on, 48 off, and we have "day truck medics" which work 40 hours a week, usually 8-10 hours a day, and don't usually work overnight.
At AMR (another service in town) they can work 8 hour days, 12 hour days, or 24 hour days.

A 24 hour shift sounds brutal, but it's not so bad. It's not like we work continuously all the time. When you have a call, you have to work, but in between calls, you can clean the station, take a nap, watch TV, or play on the internet.

It depends on where you get hired. I worked for 10 years on an ambulance service, we were stationed in the ER, so if we were not in the field, we were in the ER helping the doctors and nurses with patient care. I don't know the routine of a paramedic service that is field based. It is rewarding either way, being able to help people and once in a while save a life (it doesn't happen all the time).

Well where I am in Indiana, it is 24 on, 24 off, 24 on, 24 off, 24 on, then 4 days off and the cycle starts over again. But I know of some places that they work 12 hours. Just different with different companies.
