I have the same problem. I have not slept thru the night without waking up for several months now.
You might try taking a warm bath in the evening. Cut your fluid intake off after 8 pm or so and try not drinking any caffeine in the evening. It doesnt help me, but it might you. If that doesnt work, you may have to go talk to your Dr. about a mild sleeping pill.

Do you exercise?
I find that if I exercise a lot during the day, I'm so tired that I sleep well and only wake up to use the restroom. Give that a try and also stay off the caffeine (sode, coffee, etc) a good 5 hours before you sleep.

Alcohol before Bed,Obesity or an Underlying medical condition can affect sleep and Sleep patterns!(Not saying you have any of these-Just pointing them out)
Have you tried Tapping on your Cheek the number of Times you wish to wake up at!?
For Instance if you want to Wake up at 7am- Then Tap your Cheek(With Index Finger) 7 Times!
Works for me!

See there is Black Magic for this U need to take the Arm Pit Hairs of Monkey from Amazon Forest. Remember not the Cool Monkey but the Wild one. Then put it into your Drain for month rapped in Cloth. when it smells unbearable mix in water and drink it.
That's how you can get rid of this.
Think of it in night and hope you can get some sleep being scared to take such challenges.

Oh, most probably, you're sleeping on the wrong side of the bed. But then, try acupressure. Press your fingertips to release such pain so as to enable your brain to receive it. This might help. I am sure, you're not too old already because this happens to the oldies ;)

Take a sleeping pill before sleep. An over the counter pill. But dont take the recommended dose, take 1/2 the dose. Just enough to keep you asleep, but not enough to knock you out.
