usually not assoc with heart attack. Would likely be deep aching pain in the upper area near the shoulders.

There can be heart attacks that have back pain as a symptom. It's more likely in the elderly, in women or people with diabetes. The pain would be the same type as if it were in the front (i.e. it would come on with exercise or stress, be crushing in nature, have associated anxiety, trouble breathing and sweating and would lessen with rest).

However, heart problems are a very rare cause of back pain. There are many other things I would think of first.

If the pain is intolerable or if you have any of those other factors described above, you should call 911 and get to ER now. If not then check with your doctor in the morning.
