VERY good question, one I've thought about on occasion.

I think that when people cuss out of a "knee jerk" reaction (ie hitting thumb with hammer, just seeing a car wreck), it is more about the expression of shock, disbelief, and anger than anything. This is where we train ourselves to use a word, and it often isn't a nice word. In these cases, I think it's just fine to retrain ourselves with a non-offensive word.

When people cuss as part of their normal speech "that effin' clerk," that's when I start to wonder what's the point? We get lazy with our speech and instead of saying "that rude and lazy cashier" we say "that b*tch" or whatever.

I think context and the meaning of the word are both important factors. For me personally, I've been known to use f**k, b**ch, sh*t, etc but I don't use "God" "Jesus" or "damn" cuz those words have very real and personal meanings for me.
