The fear of asking someone out if fairly normal. However, the most important thing in any relationship is communication. You'll never know if you don't ask. I would say that the most personal approach you are comfortable with would be the best. I would probably steer away from asking someone out in a note. For one, asking someone either face to face; or even on the telephone means that there can be dialog. Writing a note is much less personal and gives room for that person to have more questions than answers. You have nothing to lose by trying, so give it a shot. If it is meant to be, it will happen. If you are scared to do it in person follow these steps...
1. Find a friend that can get some information from him. You want to have them see what his feelings are towards you.
2. Once you get the thumbs up that he likes you, ask him in person.
3. If you are still nervous, then go the note route. Guys like to get something out of the ordinary.

Does he like you too? If so, then I would ask him face to face. If he barely knows you then I would start talking to him more and hanging out with him. Then ask him face to face. I definately wouldn't ever ask him in a note.

Amazing. Men do it every day ... and they mostly survive it.
If something is going to go wrong (and that is very unlikely), it
is more likely to happen if you leave around evidence (like a note,
e-mail, etc). You're better off doing it face-to-face or over the
phone, since there will be no evidence of it other than your
memories of the ... event.
However, you're over-thinking it. Just do it.
Hi: Would you like to go with me to XXX on YYY at ZZZ?
Yes: Great! How shall we get there?
No: Oh well - thanks anyways.
Believe me - most men will be thrilled. Those that wouldn't
be ... well, this is not a bad way to find them out and start
ignoring them right now.

I think as much as you are scared to ask , some women dont realize how much power they have over men ,when you ask him ,he is going to be just as freaked as you !! And you dont have to ask him out right on a date ,just ask him if he would like to go for a coffee or a drink some time soon ,and set it up face to face !! Then it will just turn into a date if everything goes well
