It doesn't matter if he wears underwear or not. I mean, in the long run he'll have saggy balls from lack of support but it doesn't mean he isn't clean, like some of the other comments imply. It just means his preference is to go commando. Nothing wrong with that.

I wouldn't go comando for the first time. It would seriously raise a few flags and leave the girl wondering if you did it just for convienience this once, or if its a daily trend, which is rather gross. Try it out after a couple intimate experiences. It'll be a pleasant surprise.
