It seems originally there was to be a question mark.
As in Doctor Who? But as we now know the idea was dropped. But yes it is a question. Not his name.

Hmmm............He was introduced simply as "The Doctor" on the first episode. He introduced himself in every single story this way.
He was also called Dr. Foreman.
Then he was called Dr. Who in an episode; and it was acknowledged as an error by the writing staff.

So, my answer to you is : No, it was a mistake.

He also had an alias: Dr. John Smith.
Also a nickname: Theta Sigma.

many people who meet him ask him Dr who cuz he just says im the doctor so yes its a question.
