Hamburg is made from scrapes of meat that can't be used for anything else. Ground chuck, is the part that is usually made into roast, Ground steak is ground steak, usually the round steak. It's ran through a meat grinder and packed according to what it is (hopefully, at least)

If it's lean ground beef, it comes from the whole cow, minus the feet and head. For not so lean stuff, they toss every thing in and chop it up real fine.

basic ground beef is left over meats from every cuttings and lots of fat. Nowadays you can choose leaner ground beefs from different part of cow, and it will say so on the package. Still, it's from scraps.
