I watch movies, listent to music, surf on web, talk to friends but I still have much difficult to understand large phrases. It would be vocabulary poorness or difference of accents? Anyway I am trying to improve my english language. Can you help me?

Keep doing what you are doing. You could take speech or english class at the local community college, It won't cost much, you will meert interesting people, and if you aren't interested in credits, it won't hurt if you miss a class or two. just be honest with your instructor. If you have an accent, drop it. talk like the people around you, Different parts of the us people sound different. Blend in.

May I suggest that you try reading newspaper articles aloud to a friend.
Ask your friend to correct you when you say a word wrong by telling you how to say the word correctly.
Also, have your friend tell you the meaning of the words you do not understand.

Just do it. A lot of people are bashful about using their second language. It doesn't sound like you are. Practice with everyone you meet.
