A pillow case is something that you would actually sleep on, whereas a pillow sham is something meant to be decorative, not necessarily something you'd sleep on. Pilow cases would typically be made of the same material as sheets (cotton, silk, etc.), and pillow shams can be made of all sorts of decorative materials, many times even matching the comforter exactly.

A pillow case covers the pillow, a pillow sham can go over the pillow case & is a decorative cover for the pillow - it may match the spread/comforter on the bed.

A pillow case is a simple cover for the pillow that you use while sleeping; it seldom has elaborate decorations on it. A pillow sham is a more decorative covering for the pillow that goes over the pillow case during the day; it probably matches the bedspread or comforter on the bed, and is often far more elaborate than a pillow case, with embroidery, quilting, pleats/ruffles,etc.

Sham has a border and the opening is in the middle so that the pillow can not fall out as it could with a pillow case

A pillow case is the covering that you actually sleep on. A pillow sham is usually a decorative covering that you only put on the bed once it is made. For example, I have four full size pillows on my bed. Two have pillow cases. These are the two that my hubby and I sleep on. Then I have two that have pillow shams on them. The shams match my comforter. We only put those pillows on the bed when it is made.

Sham is decorative, often has a border on all sides, has a zipper, instead of an open end, etc
Pillow case is just a cloth bag for the pillow

A pillow case is for a pillow that is used for sleeping. A pillow sham is decorative. It isn't for sleeping.

a pillow sham is for decorating on top of the bed. the pillow case is for sleeping on when you are in bed.

Not to mention, pillow shams aren't nearly as comfy as a pillow case.
