Maybe you really do need a good teacher. You can't go on here and ask complete strangers for help with an answer. Some people here will happily mislead you, and you won't know who to trust. Worse yet, you won't learn to do things for yourself once you get an answer you trust.
If I tell you that A is the correct answer, why aren't you asking me why? Because you can't on here. This is not the place for you.
Learning is questioning and answering, for sure, but it has to be much more interractive than what we have on Questions and Answers.
Here is your lesson of the day: When "it is" becomes contracted, you need an apostrophe to indicate a letter has been missed out. It is = it's. (Look at these examples:
that is = that's. where is = where's. what is = what's. etc. See a pattern?)
Examples B, C, and D all fall into this rule. Every meaning is "It Is".
The monster has a head, and it is the monster's head, therefore "its". Sally has a boyfriend; he is Sally's boyfriend, but HER boyfirend too.
James has a car; it is his car. Now, if we were to substitute his name for the pronoun "his", would it be
a) Jame's
b) James's
c) James'
d)Jameses

'A' is correct.
B: The problem with the lawn mower is that >it is> old.
C: When trying to catch fish, >it is< important to be patient.
D: >It is< easy to learn to play the guitar .....etc
So the Answer is
A: The monster reared >its< head at the tourists.....
otherwise it would read....
The monster reared >it is< head at the tourists.
which of course would be as much nonsense as 'It is easy to learn to play the guitar.'

A. The monster reared head at the tourists
There is no apostrophe in the possessive of it. Only in the contraction (it is).
