Fill in the form as instructed. Your birth certificate should have your parents' full names and possibly addresses at the time of birth. If it is only a short certificate you can obtain further details from the Register Office that issued your Birth Certificate as full details are kept there. You do not say if your parents are alive. If so then do you have contact with them? If so you could ask them. If deceased then you can obtain details of them from the register Office that recorded their deaths. If they are alive and you are not in contact with them tell the passport office the circumstances and their last known addresses as they should be able to obtain any details required from the National Insurance Records. Also you say you are a British citizen but do not say whether you were born in Britain. If not and you came as a child there should be naturalisation papers somewhere. Record all these details on a separate sheet of paper and enclose it with your application.

Your parents' names are entered on your birth certificate, if it is a full birth certificate. If you only have a shortened form, then obtain a full birth certificate from the registry of births: http://www.gro.gov.uk/gro/content/ . If your parents come from the UK, then you can apply for copies of their birth certificates. If they are dead, then their death certificates would contain details of their birth. If they come from elsewhere, then it becomes more difficult, but perhaps you can get help from the Post Office, where they are used to dealing with passport applications. What you don't want to do is get everything sent back to you because of a mistake. I hope everything goes all right and that you have pleasant travels with your passport.

As long as you write down your parents names, which you say you already know, then that should be fine. I would also apply for a birth certificate - if you weren't born in UK you may have to write to your birth country and request one. Or if you were adopted, your legal guardians might be able to provide you with some of the information you need. It may be that your adoptive parents names, and your adoption papers, are all you need to support your claim. You are a British citizen, you should have no problems. ;-)

You should find more of your parents' details on your birth certificate. If you don't have that you can get a copy from your local registrar.

You can also contact St Catherine's House in London and when you pay a small fee they will produce the details for you.

are you in contact with your parents or can you arrange contact or can a third person act for you between your parents because that's the road you should go down or its going to cost you one way or another
