I really feel he should have seen a doctor before now. If he hasn't please take him.
Also, is he under stress from something like a divorce or move?

Try not to worry and make a big deal about it. Does he do this at school? If not, he's doing it for attention at home. Give him good praise and rewards if he does do well otherwise try to ignore it.
If in doubt, see your doctor, they can do tests to see if it is a medical problem.

Are we talking number one, or number two? I would definately take him to the doctor. I doubt this is a problem a normal boy would endure due to laziness.

Please take him to the doctor immediately. This is a sign of mental trauma. Something has happened at school that has upset him, or someone has been messing with him. This is a sign of sexual abuse. Hopefully, that's not what it is, but don't wait, take him NOW!!
