Yep, if she lives in your household and drives the vehicle she has to be either included or excluded. There are no exceptions.

She is covered even if she drives without you. You do, however need to add her. If she has an accident and you haven't, the insurance will add her anyway. Not sure why you think whether she drives with you makes a difference.

You have to add her or exclude her. She is a resident relative/household resident.

Without her on the policy, they can deny coverage for a loss she is involved in while driving the car.

If she will NEVER drive the car, exclude her from the policy so you don't have to pay for her to be rated.

Yes, add your wife to your policy. You want to have her covered in an emergency or if she backs it out of the driveway. Depending on her age/driving history, she may not even affect your rate.
