I usually just hold it; they seem pretty on-par with port-a-potties. Just don't hold it too long, or you'll not be able to pee after a while (from cramping your muscles too much to keep it in) and you'll involuntarily "let it all go." My psychology teacher had a similar problem ;)

I had to go number 2's on a trip from Melbourne to LA. Problem was that I had to go just before we made our final approach and most people were up and about. I lined up for about 5 minutes, went and then copped so many dirty looks on the way back to my seat that i just stared at the little tv for the rest of the trip. At least the guy who kept knocking at the door had to deal with the horendous smell i left for him!!!

Airplane toilets are almost as small as airport toilets. I think they keep them small so no one gets too comfortable.
The toilets are there to do the job. They do their job and have no frills.
Of corse you hate airplane toilets, we all do.

I try to go before the flight, and if not I try to hold it till I get to where I am going.
