Why would you want to forgive your ex. He is your ex for a reason. Unless you really love him and you can handle him doing whatever it was to you again then just talk to him. Let him explain and apologize and make sure he does it well. If you can work through the problem then good for you two.

In order to move on you need to come to terms with it and just be the bigger person and forgive. Just because you forgive, you don't have to forget.

If cheating was involved you don't anything else maybe depends on what it was and if you both still have feelings toward each other and not one sided.
