It depends on your plan. You need to call the number on the back of your card and ask if you have mental health benefits. If they say yes, ask them to send you a list of in-network providers.
Be sure to ask about the amount of your copay (if any), and how many sessions per year are covered.
You can also ask if you have out-of-network mental health benefits, and if so, how much/what percentage do they reimburse, and how many sessions/year. This enables you to go to any therapist you want, pay the therapist yourself, and get partially reimbursed by your insurance.
Lastly, if you do not have benefits, look for a therapist who has a sliding scale fee. This means they are able to lower their fee based on your financial situation.

Some plans do. Look at your insurance card - there should be a phone number for Mental Health, if not call member services. There will more than likely be a list of "preferred" providers. Many plans are self-referring for mental health - but you probably will pay more than your standard copay. (When I had Oxford, I had to pay 50% of the bill.)

Depends on your health insurance. In any case most insurance these days don't pay for enough sessions to be effective. Depending on your income, you might be able to qualify for free mental health therapy (I assume that is what you mean) so call your local MHMR (Mental Health Mental Retardation) office and see if you qualify.

Yes - the usually cover some basic therapy,. But the only way to see how far they will cover is to review your contract and contact the agent.
