Oh how sad, Please be delicate with this child...saying or doing the wrong thing can scar them for life and forever shape their self esteem, the way they deal with relationships...etc.
I don't have advice on what you should do, I just know what you should not do. Never critize them..

Find fun things to do outside together, and start cooking healthier and don't have the junk food in the house.
I know my children lost weight without even realizing it when I started taking them with me on walks and they were eating healthier foods because I was on a healthy diet and didn't keep the junk in the house. They can only eat what you allow them to eat or keep in the house available. Yes I know you have not control of their diet when away from home, but what is at home makes a huge difference.
Show by example and always make them feel loved for who they are.

My daughter had the same problem with her then 6 year old son. Not only was he overweight but his cholesterol was high too.
The first thing she did was put him on cereals like Cheerios. Then she let him pick a sport. His first choice was golf. So she signed him up for gulf with some other kids his age. She rented the clubs and didn't spend much money. He ended up getting hit in the head with a golf club requiring 7 staples. Then he tried baseball. Again she didn't get in over her head and let him join. He played for one season and then didn't want to play another. She said fine, what would you like to play. He chose flag football and really seemed to enjoy it, but at the end of football season, roller hockey season started and he wanted to join. He is now playing for his third season and is loving it. I waited until he decided if he was going to stay in or not and then I got him a very nice pair of indoor roller blades for him to wear during his games.
He has lost a lot of weight and his cholesterol is fine. It took a lot of patience and time on my daughter and son-in-laws part but everything has worked out and my grandson is much more active even at home now.

Lead by example. Eat healthy food in moderate portions yourself, to show them what is appropriate. Get exercise yourself by walking instead of driving to the store for instance, or parking further away in the parking lot when you go somewhere, to get in more steps or take the stairs instead of the elevator. Put in a workout tape or dvd and work out. Go for walks in the evening after dinner instead of sitting in front of the TV. Kids learn by what they see, not what they hear.

Arrange a trip to the local morgue and show 'em some folks who stroked out or dropped over from heart disease or high blood pressure due to being morbidly obese.
