Immerse yourself in English. Read only English, except for the dictionary that translates back and forth with your native language. Speak only English to the extent possible, and try to engage your family in learning it with you. Speak to native English speakers to the extent possible. Travel in England or other English-speaking countries. Subscribe to English-language magazines. Write letters and emails to native English speakers and ask for their feedback if something you say is hard to understand.

Good luck! The ultimate, of course, is to get good enough in English that you can attend an English language college and really learn what you need to know.

I'd suggest playing an online game with a forum or a chat room where you can read and participate. People are usually pretty understanding if you tell them you are learning, and you will pick up the language faster if you have to use it :)
