If your child is in physical therapy at school then they had to have had an IEP to have it set up. You need to request a new IEP in writing from the school psychologist. They will set up a new IEP and that is where you request that your child be taken off of physical therapy.

The first answer is correct. You contact the principal and school teacher and request a meeting with the Child Study team or diagnostic team to review your child's IEPC. At the meeting tell the committee that you request to have services discontinued.
You will be asked to sign some papers and then the services will be stopped the next day.
By the time you request the appointment, they must legally act and set up a meeting with you within thirty days of your request.
