If the blood is not visible to the naked eye, it is known as microscopic haematuria. If you have any urinary tract symptoms (which you obviously have or you wouldn't have been referred for a cystoscopy) then the first line of non-invasive investigation is testing the urine. The dipstix are very sensitive to blood, protein, ketones and glucose. 4++++ is the highest reading for blood whether or not it's visible to the naked eye in your urine. The significance of this varies - occasionally further investigations may not reveal anything. The most common reason in women is a urinary tract infection (UTI) which is treatable with the right antibiotics. UTIs can range from non-specific urethritis (inflammation of the urethra) and cystitis to pyelonephritis which is an infection of the kidney itself. Other reasons include narrowing of the urethra, kidney or bladder stones or (rarely in younger people) bladder tumours. All of these are treatable. If nothing shows up on cystoscopy you may need more detailed investigations to rule out kidney problems. In the course of my work as a pre-op assessment nurseI have met several patients who always have a microscopic amount of blood in their urine, have been investigated fully and no cause found. It's just the rough labstix measurement which goes from 0 to ++++ for anyt of the urinary substances measured eg blood, protein, sugar. It's the result of your cystoscopy that's significant.

A severe bleeding coming from the urinary system. If the cystoscopy is abnormal, it comes from the bladder probably.

4 ++++ blood means you have a lot of blood in your urine. Your Urine would look bloody.
Means you may have an infection, bladder or kidney problem.

4++++ means the amount of blood in your ruine detected by distix. It was significantly more.

4++++ indicate the sugar level, with that sugar is being excreted through urine
