Dirty depends upon context.

it does not have to be dirty. the frase has a lot of meanings but it usually means that someone does sth very good. "hace" means "to do" and "muy bien"= very good.

By the way, this phrase is used sometimes with a double meaning --> He/she make love very well.

It means "you do it well" or "you do it great" if he is refering to you , if he is not it will mean "she/he does it great or well"
