if i have back or neck pain and why does it affect those areas?

I have to say that I have had acid reflux, and David's answer seems dubious to me. (No offense, Dave) Vinegar is acidic, Acetic acid specifically. Seems to me if you drink it, even with the Mother intact (yes there really is a Mother) it'll simply worsen the condition. Acid plus acid only makes more acid.
When mine got bad, and I couldn't get Prevacid, I started drinking milk again. I had developed a lactose intolerance, but kept drinking the milk, and after a few months, the intolerance went away, and the reflux was under control. You might also avoid tomato products, colas, and orange or pineapple juices.
As to your back pain, it could be sympathetic, or it could be from your posture, which could be caused by tension in your back because your muscles are tight and tense from the reflux pain.

This is a serious answer for your acid reflux. Drink several tablespoons of vinegar in water every day and it somehow neutralizes it. It must be natural vinegar, probably from a health food store because it has to have "mother" in it. (I know it sounds like a joke but isn't) Mother is removed in most vinegars though the way it's processed but it has natural healing properties and after a week or two you should see a big difference. I can't help you with the other though.
