My dad is 68 years old. He had brain hemorrhage 4 years ago and went under surgery. He was fully recovered. Since then he quit smoking and was on healthy diet and had regular exercise. His blood pressure and blood work have been normal. However, he just had a 2nd brain hemorrhage after 4 years from the 1st one. This one is minor than the previous but occurred at the same spot. His doctor can not diagnose the cause. Does anyone have any clue of the cause and treatment? Thank you.

It's due to the weakening of the walls of the blood vessels. It's that you can repair the blood vessels during surgery, but it's not going to be as strong as before. Kinda like how you can repair parts of your vehicle, but nothing is as good as getting a new one altogether. There might also have been some scarring in the area, so even if it is that the blood vessel was repaired, the scar tissue would have blocked the area somewhat, so that would make the same thing happen at an even lower systemic blood pressure, because his intracranial blood pressure would be higher in that spot itself.

it looks like he have an aneurysm, a small one that bleeds every now & then.
another possibility similar to aneurysm is called AV-Malformation
What did he have in the first surgery? did he undergo aneurysmal ligation?
He may need visualization of his cerebral arteries either via Cerebral Angiography or MRI (MRA).
