Of course the museums and shows are great, but you will also want to get some fresh air.

1. Take a horse-drawn carriage ride in Central Park and a rickshaw bike beyond the park. The rickshaws are easy to flag down in Midtown, and they can go to virtually any street in the city, not just around the carriage trail in the park. They can be a bit pricey, but try to negotiate.

2. The buses are quite good in New York. Most tourists never take them, but if you have health problems, avoid the subways (especially during rush hour) and stick to the buses. On that note, a chartered bus tour is a great way to see the whole city in a day or so. Try the double-decker!

If you are heavy then I'd avoid the Broadway plays because the seats are really tight, but if you have a wheelchair then you can look for wheelchair areas and enjoy it from there. If you are not big, then definitely go to the theater without a wheelchair and you can fit in the seats. I agree that the museums are a good place to go (but not the cloisters because they are up on this cliff and I think you have to climb the stairs to get there). Any of the restaurants are good to go to. Take one of the bus tours and see the city that way instead of walking. You can also go to the top of the Empire State Building. Take the Circleline tour (boat tour around Manhattan) and from there you can see the Brooklyn Bridge and Statue of Liberty and Lower Manhattan. Go to Lower Manhattan to see ground zero and also you can see Battery Park and South Street Seaport. I think Central Park is a must, but if it's too much walking then don't bother, because I think a wheelchair would be cumbersome on some of the paths.

Have a good time!

I'd start at a very slow pace in a workout gym. Like going to it twice a week or so. In New York one thing you definately don't want to do is go out running or jogging because all it does is kill your lungs even more from all of the air pollution.
