If you're a Canadian citizen go to any passport issuing office in town (check either your phone book or the web). If you're a citizen of another country and live in Canada go to the consulate of your country.

Only Canadian Citizens are entitled to a Canadian Passport. As the writer above me stated, there are Passport Offices in every major Canadian city. You can also pick up an application at the post office or go to the Passport Office website. Once you have the application form you can also apply by mail. Bear in mind though that you have to send along the original proof of Canadian citizenship (no copies).
If you live in a very remote area you can also send your application to your local MP. It has to be very remote such as in the Arctic or Far North.
Above all make sure you submit your proof of Canadian Citizenship.
