Some people with a Phd do not use the title Dr. because they don't want to be mistaken for a medical doctor.

Some MDs use Mr. in their personal lives because they do not want to be asked for free medical advice.

It is rare for a doctor in a practice to ask his patients to call him Mr. In that case, I would have to wonder if you are dealing with a real doctor.

In the past, you did not need to be a qualified doctor to be a surgeon. So, people who were qualified medical doctors were called Dr, and specialist surgeons were called Mr. (as they had not formal medical qualification)

The tradition has continued that when a doctor becomes specialised, they are referred to as Mr.

Sadly, I know this from an episode of 'the Cosby Show'. Bill Cosby's character was a doctor but he was referred to as Mr. I checked out medical history and he was called Mr Huxtable as he was a specialist surgeon.

Outside of work, they are simply other people. It is not unusual to want the anonymity of dropping the title in different situations. In many cases, it can be a matter of privacy. You have no idea how many people will come to you outside of work with all their health complaints if they learn you are a nurse or doctor. When your friend's kids get sick, they call YOU in the middle of the night before the pediatrician. It is just a matter of preference and privacy.

You're English, right? Cause in America, every doctor practically DEMANDS to be addressed as Dr., even people who only hold doctorates in their fields, like Doctor of Literature.

Prehaps esepcially if it is a young Dr. he may feel that it makes him sound older or he feels he is putting on airs, just refer to him as he asks. My dr. once told me to stop saying "Im not Dr. Coder, thats my father, I'm just Mr. Coder or Brian." so it works
