Reflectors. The small hole was so the doctor could see through it while reflecting a bright overhead lamp into your cavities. Falshlights replaced them eons ago.

Tiny lights, that reflected from the reflector it shined from, to give the doc a better view of the specific scene.

I don't know the proper name but they are lights so if the doctor couldn't see something very well then he could turn the round shiny thing on his head and it would light up.

I dont know did he use them to check the trauma to your brain since you seem to ask the same question over and over and over again.
