Usually it's a result of immaturity, regardless of age. You'd be surprised how shy most guys are, even if they act out-going and pompous. If a guy you know can't make up his mind, try helping him out by making it clear how you feel (guys are also absolutely terrible at reading signs, as a guy I can vouch for this). This will give him the confidence he needs to decide one way or the other.
