Most of the time it will be in their own regional accent. However If the person associates certain accents with certain personality types, they may hear different aspects in different accents. Most of the time a person hearing voices is there subconscious trying to protect them from some trauma. So who are you more likely to take seriously about a threat... someone with a californian surfer dude accent or say the stereotypical italian mob accent... or some such. See what I mean?

When I'm reading, I always hear the words in my head in my native accent. I suspect most people do. I once worked with a Bulgarian girl, who from the age of 18-24 lived in France and at 24 moved to the UK. I asked her if she dreamed in Bulgarian or French or English. She said French, but the longer she lived in England, it could occasionally be in English. So, in short, I think it's what you are most submersed in at that time in your life.

Most people hear voices like they do in their dreams; the voice matches the circumstances. If the person hearing the voices is mentally unbalanced, all bets are off.
