On 07/29/2006 at approximately 1230pm on Yahoo Answers Victim 1 Okchico was contacted by Suspect 1 Sal1200 in regards to a report writing sample. S1 did write "Police report writing sample?." V1 Okchico did respond to the question though the question was vague and not detailed. V1 Okchico did want more details of what S1 Sal1200 was looking for but details were not available. V1 Okchico does not want to press charges at this time and only wishes there to be a record of this incident. V1 Okchico advised to contact police if further assistance is neccessary. No further details.

What are you wanting? Most police reports include the names, addresses, and phone numbers of all people involved; date, time, and place of incident; a full description of the event; a diagram (if useful); and a report number. They usually print to make their writing more legible.

Here is a brief fictional police report I'll throw out from the top of my head: On Sunday, April 26th, 2009, I was assigned to the evening shift. At approximately 00:15 hours, I responded to 123 Road Street in reference to a domestic in progress. Dispatch advised that an altercation between a male and female was in progress and believed no weapons were involved. Upon arrival, I received the following information. I spoke with Jane Doe, who advised that she and her husband were arguing over financial discrepancies. Mrs. Doe believed her husband, John Doe, had been spending time at a strip club. When she confronted John Doe of her suspicions, he struck her on the left side of her face with his closed fist. I observed swelling and bruising around Jane Doe’s left eye. Jane Doe advised that she fled from John Doe and locked herself in the bedroom and proceeded to call 911. I made contact with John Doe, who advised me that Jane Doe had accused him of spending their money at a strip club. I inquired if he had struck Jane Doe, to which he replied no, I’d never do such a thing. John Doe avoided eye contact and rubbed the back of his neck as he responded to the question. I asked John Doe to show me his hands. I observed that the knuckles of John Doe’s right hand were red and slightly swollen. Pictures were taken of Jane Doe’s left eye and John Doe’s right fist for evidential purposes. Jane Doe also provided a written statement of her accounts. John Doe was placed under arrest for assault in the second degree. He was subsequently released from the Dorchester County Commissioner’s office on his own recognizance. If this were on a charging document in the state of Maryland it would end with: “All Events occurred in the Town of Hurlock, Dorchester County, Maryland.”
