When you apply for the tourist visa to the US, you will be submitting copies of your marriage certificate and husband's and child's US passports to the US Consular officer. You will apply, and the visa will be issued, in the name on your Philippine passport.

You should have no problems, but it would not hurt to have a copy of your marriage certificate when you travel. In the future though, unless there is a reason you choose not to, it will be simpler if you change your name and have a new Philippine passport issued in that name. It will also be a benefit if you decide you want to pursue immigrant status in the US.

If you use your husband's last name in the Phillippines, then yes you should because when they do background checks they will be looking for whatever name you use legally in the Phillippines.

Try getting a new passport with whatever last name you use regularly, if you just use your single last name for everything in the Phillippines, then don't bother, you are not forced to have the same last name as your spouse to get a visa.

Changing your name beforehand may complicate things - your birth certificate will have a different name.

I'd contact the US embassy for their advice.

If you took your husband's name when you married, then yes - you need to update your passport (and any other I.D.).
