Think of the mafia boss as the general in an army. Then realize he just has to think and not actually do anything. If he wants something then he delegates to the next level down who is the army equivalent of a colonel. That equivalent who has to worry about actually making things happen is the mafia underboss.

It is all a matter of rank. It has nothing to do with wives or lady psychiatrists ending up under the boss, no matter what you saw on the Sopranos.

Middle to upper management of the mafia business. He'd report to the top boss, head of the family. He'd also be responsible for a portion of the business, and would have subordinates who report directly to him.
